You will need to complete TWO forms to register with us (‘Register’ and GMS1).
Please download the relevant forms below, fill them in and email your completed forms to barccg.stamppatient@nhs.net
You will need to complete one of these forms (Adult or Child under 16y) for each person you wish to register with our practice. Those fields marked with an * are required.
Finally, download and fill in the GMS1 form and email to the surgery along with a copy of your photo identification and a copy of your proof of address (this needs to be dated within the last three months).
Please note: Failure to provide completed GMS1 forms, registration forms or copies of your identification/proof of address will delay your registration.
GMS1 formWe aim to process your registration within 5 working days.
To complete your registration you will be contacted by our administrator who will schedule a short video consultation to verify your identification, this will take less than five minutes and allows us to confirm you identity without you attending the practice.

Important update regarding the Covid-19
We have now invited ALL patients OVER 65 years old and patients that are CLINICALLY EXTREMELY VULNERABLE (shielding) for their FIRST dose Covid Vaccination (not including pregnant women and those under 16 years of age).

Important update regarding the Covid-19 vaccination programme.
The NHS is prioritising the rollout of the Covid-19 vaccine.
Following extensive trials, we now have safe and effective Covid-19 vaccines that have been approved in the UK.
The first phase of the NHS COVID-19 vaccination programme has begun. Vaccinations are being delivered according to priority groups identified by the Joint Committee of Vaccination and Immunisation. Initially we will be prioritising those aged 80 and over, care home workers and residents, and frontline health and care staff. Patients have started receiving vaccinations, as more vaccine supply is produced and available more patients will be contacted and asked to come forward for their vaccination. The vaccinations will be delivered through a limited number of Hospital and GP Hubs.
